Please take note of
the following suggestions to help you prepare the scholarship in the way it
should be:
1.
Read through
the entire form before starting. Make an
extra copy so you can have a scratch and a finished product.
2.
This should
be type written or print neatly. Make
sure your printing is legible.
3.
Please put
the items/pages in the order specified on the scholarship.
4.
DO NOT type
the page for your personal statement, cut around it and glue it on the proper
page in the scholarship form. With
modern technology we should not have to do this.
5.
Number the
pages as directed on the scholarship form.
6.
Put your
name on each page and make sure YOU SIGN the completed form.
7.
DEADLINE is postmarked by December 31sth, 2003
8.
Send
completed form to: Julie Rieckman,
9.
DO NOT allow
your school to give you a transcript printed on recycled paper, this is
important enough for you to use a good sheet of paper.
10.
Be
consistent in your plans for the future and what the people have written about
you in the letters of recommendation. An
example: You want to be a Forest Ranger
(Smoky the Bear is your Hero) and 3 of 5 people who wrote letters for you
thought you were going to be a Pharmacist!
11.
The most
important item is to SIGN THE
APPLICATION
If you have any
